Please arrive early so that we can complete your initial paperwork. Also, please bring the following:

• Insurance card if applicable
• List of current prescriptions and/or over-the-counter medication, including dose and frequency
• Information about patient's medical and surgical history
• Recent test results, x-rays, or relevant records
• Completed new client/patient form (PDF)

Summit Pet Hospital accepts the following payments for service:

• Cash
• Checks (established clients)
• Credit Cards (Visa, MasterCard, Discover, and American Express)
• We also accept CareCredit® (no interest payment plan). Clients may go to www.carecredit.com to apply.
